Suggested Bequest language

Option 1:
To make an unrestricted estate gift that will be used to support the mission and work of Affinity Health System in the Oshkosh area region and throughout the Fox Valley region. Foundation Trustees determine priorities for use of the gift based on current health system priorities.

I bequeath to the Mercy Health Foundation, Inc. ("Foundation") or its successor organization ___________ percent of the property described herein or the remainder of the property which I own at the time of my death, for the purpose of providing an unrestricted bequest. The use of this bequest may be directed by the Foundation for charitable health purposes of benefit to the area served by the Foundation.
Option 2:
To make an unrestricted estate gift to benefit Mercy Medical Center that will be used to support the mission and work of Mercy Medical Center in the Oshkosh area. Foundation Trustees determine use of the gift based on the current priorities of Mercy Medical Center.

I bequeath to the Mercy Health Foundation, Inc. ("Foundation") or its successor organization __________ percent of the property described herein or the remainder of the property which I own at the time of my death, for the purpose of providing a Mercy Medical Center unrestricted bequest. The use of this bequest may be directed by the Foundation for charitable health purposes of benefit to the area served by the Foundation.
Option 3:
To make a restricted estate gift that will be used to support the mission and work of Affinity Health System in the Oshkosh area and throughout the Fox Valley region. Foundation Trustees direct the use of the gift based on an area of health interest.

Please consider consulting with Foundation staff for suggested wording to establish restricted bequests for an area of interest.

I bequeath to the Mercy Health Foundation, Inc. ("Foundation") or its successor organization ___________ percent of the property described herein or the remainder of the property which I own at the time of my death, for the purpose of providing a restricted bequest. This bequest may be used by the Foundation for charitable health purposes consistent with my interest in _________ for benefit of the area served by the Foundation. If support of this area of interest is no longer needed or appropriate, please direct my gift to ______________ or have the Foundation redirect my gift for charitable health purposes consistent with my interest.
Option 4:
To make a restricted estate gift to benefit Mercy Medical Center that will be used to support the mission and work of Mercy Medical Center in the Oshkosh area.

I bequeath to the Mercy Health Foundation, Inc. ("Foundation") or its successor organization __________ percent of the property described herein or the remainder of the property which I own at the time of my death, for the purpose of providing a Mercy Medical Center restricted bequest. This bequest may be used by the Foundation for charitable health purposes consistent with my interest in _________ for benefit of the area served by the Foundation. If support of this area of interest is no longer needed or appropriate, please direct my gift to ______________ or have the Foundation redirect my gift for charitable health purposes consistent with my interest.
Option 5:
To establish a Permanent Endowment Fund for 1) unrestricted use (Affinity Health System), 2) unrestricted use to benefit Mercy Medical Center, 3) restricted for Affinity Health System area of interest, or 4) restricted for Mercy Medical Center area of interest.

Please consider consulting with Foundation staff for suggested wording to establish a bequest for permanent endowment.

With this bequest, I wish to (select one): 1) join others in a permanent endowment fund or 2) establish a permanent endowment fund that shall be known as the: _______________________________________________________________. * Foundation Trustees shall have full authority and discretion to invest assets of the endowment fund consistent with the Foundation Investment Policy, Articles of Incorporation and By-laws. This includes the power contained therein for the Foundation Board of Trustees to modify any restrictions or conditions if in their sole judgment (without approval of any trustee, custodian, or agent, unless specified in a Foundation gift agreement) such restrictions become, in effect, unnecessary, incapable of fulfillment, or inconsistent with the charitable needs of the area served by the Foundation.

*    A $10,000 minimum gift is required to establish a permanent named endowment fund.
